The first edition of this book was published in early 1988 by Prentice-Hall, see Zbl 0671.65063. The authors pursued only a limited revision consisting of simply correcting small mistakes (mostly typographical). The path of limited revision is reflected in the list of references, which remains current only up to 1987.
